Client Intake Policy & Disclaimer
Please review the intake guidelines and disclaimers that apply before submitting a CaseBridges request.

1. Purpose of the Intake Form
The CaseBridges intake form is designed to allow individuals seeking mental health or behavioral health services to submit limited information describing their general service needs.
Information submitted through the intake form may be made visible to licensed mental health professionals who participate on the CaseBridges platform.
These professionals may review the submitted information and independently determine whether they wish to contact the individual seeking services. Submission of an intake form does not create a provider-client relationship with CaseBridges.
2. Who May Submit an Intake Form
An intake form may be submitted by:
- an individual seeking mental health or behavioral health services
- a licensed professional assisting a client
- a case manager or social worker assisting a client
- a hospital discharge planner or care coordinator
- another authorized person submitting information on behalf of a client
Individuals submitting information on behalf of a client represent that they have obtained the client's knowledge and consent. CaseBridges does not independently verify whether such authorization has been obtained.
3. Information Requested in the Intake Form
The intake form collects only limited information necessary to allow providers to determine whether they may be able to assist.
Information that may be requested includes:
- email address
- preferred language
- insurance provider (name only)
- general reason for therapy or services
- ZIP code
Users should provide only general descriptions of service needs.
4. Information That Should NOT Be Submitted
The intake form is not intended to collect sensitive medical or personal data. Users should not submit:
- Social Security numbers
- insurance ID numbers
- medical records
- diagnostic documentation
- therapy notes
- psychiatric records
- payment information
- credit card information
If sensitive medical information is shared after contact with a provider, that communication occurs directly between the provider and the individual.
5. How Intake Information Is Used
Information submitted through the intake form may be used to:
- create a case listing visible to participating providers
- allow providers to review service requests
- allow providers to determine whether they wish to initiate contact
- facilitate potential connections between individuals and providers
CaseBridges does not assign cases to providers. Providers independently decide whether to contact individuals based on the information provided.
6. No Guarantee of Provider Contact
Submission of an intake form does not guarantee that a provider will review the listing or initiate contact. Provider response may vary depending on factors such as:
- provider availability
- geographic location
- insurance compatibility
- clinical specialization
CaseBridges does not guarantee that services will be provided.
7. Independent Provider Relationships
If a provider contacts an individual after reviewing an intake submission, any professional relationship that may develop occurs directly between the provider and the individual seeking services.
CaseBridges does not participate in scheduling appointments, treatment decisions, billing arrangements, or clinical services. CaseBridges is not responsible for the actions, conduct, or services provided by licensed professionals.
8. Platform Role
CaseBridges operates solely as a technology platform that allows limited information to be shared with licensed professionals who may be able to assist.
CaseBridges does not:
- provide mental health services
- diagnose conditions
- provide treatment
- supervise providers
- assign providers to clients
All professional services are delivered independently by licensed providers.
9. Emergency Disclaimer
The CaseBridges platform and intake form are not intended for crisis situations or emergency mental health needs. If you are experiencing a mental health emergency, thoughts of self-harm, or a crisis situation, you should immediately contact:
- 911
- local emergency services
- Suicide & Crisis Lifeline (988 in the United States)
CaseBridges does not monitor intake submissions for emergency situations and cannot provide crisis intervention.
10. Privacy Considerations
Information submitted through the intake form will be handled according to the CaseBridges Privacy Policy.
Users should be aware that once a provider reviews intake information and initiates contact, further communications may occur outside the platform. CaseBridges cannot control or guarantee the privacy practices of independent providers.
11. Limitation of Platform Responsibility
CaseBridges provides the intake form solely as a tool to facilitate informational connections between individuals seeking services and licensed professionals.
CaseBridges does not guarantee provider availability, response, or treatment outcomes. Users acknowledge that participation in the platform occurs at their own discretion.
